Subcontractor's Application for Payment Continuation Sheet for all contracts over $20,000 (Excel File)
Conditional Lien Waiver (Word File)
or Final Lien Waiver (Word File)
If your contract with Hendrick exceeds $100,000, lien waivers are required from all of your lower tier subcontractors and suppliers who have contracts with you that exceed $5,000 when your contract with Hendrick reaches 50% completion and upon substantial completion. Please include the following lower tier lien waivers with your application for payment.
